Nurse and carer invoice template for agency shifts and private care

Nurses and carers on an ABN invoice two kinds of customer. Agencies, hospitals and aged care facilities are invoiced per shift from an approved timesheet, and that supply is to a business, so it carries GST once you are registered. Private clients who receive nursing care in their own home are different: nursing services supplied to a patient can be GST-free, and the invoice should mark those lines accordingly.

Facilities pay on 14-day terms and need your AHPRA registration number, the shift dates and the supervisor who signed off. Private clients and their families usually pay within 7 days.

What must be on a tax invoice

The ATO's list. The template lays each one out for you; it is general information, not tax advice.

  • The words "Tax invoice" (an invoice without GST can just say "Invoice")
  • Your identity: your business or trading name
  • Your ABN
  • The date the invoice was issued
  • A description of what was sold, with the quantity and the price of each item
  • The GST amount, shown separately or as "Total includes GST of $X"
  • Which items carry GST if some do and some do not
  • For sales of $1,000 or more, the buyer's identity or ABN as well

  • Your AHPRA registration number and the shift dates from the approved timesheet
  • Which lines are GST-free care to a patient and which are taxable supplies to a facility
  • The client's name and address for private care, and the facility's ABN for shifts of $1,000 or more

Questions about invoicing

Is my nursing income GST-free?

It depends on who you are supplying. Nursing services supplied to the patient by a registered nurse can be GST-free. A supply of your labour to an agency or facility is a supply to a business and is taxable once you are registered. Many nurses have both on the same invoice, which is why this template marks each line.

Do I need to register for GST?

Only once your GST turnover reaches $75,000 a year, and GST-free supplies still count toward that turnover. If you are not registered, switch GST off on every line and the document is titled "Invoice".

The agency issues me a recipient-created tax invoice. Do I still invoice?

No, not for that agency. An RCTI replaces your invoice for those payments under a written agreement. Keep each one for your records and use this template for any client that does not issue them.
